You will be asked to complete a self-assessment questionnaire before you attend your first therapy session. It's not mandatory but completing the questions will help both you and your therapist to get a head start in your relationship, and help the therapist to prepare in advance of your first session.
The initial self-assessment
The initial self-assessment contains three sections:
a space for you to let your therapist know more about what you'd like help with and any relevant history or treatment as well as what you'd like to achieve from therapy
two short multiple-choice questionnaires about how you have been feeling over the last couple of weeks
Ongoing self-assessments
You will receive a new self-assessment questionnaire after every six sessions of therapy you attend. This always includes the same short questionnaire about how you have been feeling over the last two weeks.
Once you've completed a self-assessment your answers will be made available to your therapist.
Why are self-assessments helpful?
Completing a self-assessment is an opportunity to reflect on how you have been feeling lately and to notice how this changes over time. You may wish to discuss your answers and anything that arose from the process with your therapist so that your treatment can address any areas of difficulty you are experiencing.
When will I receive a new self-assessment questionnaire?
You will receive a new self-assessment questionnaire after every six therapy sessions you attend. You will receive a notification by email or you can find the new self-assessment link in your dashboard. Completing a self-report questionnaire is optional.
