What information are clients asked to share in their intake forms

The initial self-assessment questionnaires for clients ask some further questions in addition to the PHQ9/GAD7 multiple-choice questions

These questions allow the client to share more detailed information about what they are seeking help with, any relevant past history, medical history, and information on their current work and home circumstances. 

The additional questions asked in the initial self-assessment questionnaire cover the topics listed below :

  • the issues the client is seeking help with

  • their hopes for therapy

  • if they have already tried anything to improve the situation

  • are any other professionals involved, or have they been involved in the past?

  • have they suffered any trauma, important change, or loss

  • questions regarding past and current self-harm, and harming others

  • if they have any current or previous physical or psychological issues or diagnoses, and any mobility or other issues for the therapist to be aware of

  • their employment status, responsibilities and hours

  • their home circumstances, and if they are happy with these

  • their hopes for the future

  • GP details if they consent for their GP to be contacted in the case of an emergency
